I manage to find values for Killzone: Shadow Fall (CUSA00002) v1.81, tested & finded in 5.05+HEN 1.7. All values are located in one game process:

anon:00000072bef4[2]-3-210400000-131072KB

Point is that you'll need search values again after game restart, and in each chapter separately (addresses changing every restart & in every chapter). Thus maybe making cheat table is useless.

1) Health starts from 250 float/exact value, make in decrease/increase etc. and can be locked, game works fine.
2) Ammo is 2byte both for 1st and 2nd ammo
3) HL/adrenaline & grenades is in 4byte values.
