Since their last PS4 1.76 UI Mod update and following the 4.05 PS4 Kernel Exploit release, today the PlayStation 4 developers at e✘treme-Modding.de unveiled a PS4 4.05 UI Mod Alpha 0.13 Custom Home Menu with some fresh screenshots and details below.

Visual PS4 UI Mod for 4.05 (English/French)
1. Install Firmware 4.05 (initializing)
2. Enable FTP Server and copy the content of the Mod_UK/Mod_FR folder to the root of the PS4 HDD
3. Restart the PS4
Info: The Games folder in the Home Menu appear if you have installed CUSA00851 and the app.db work only if your System Language is English (United Kingdom) or French.
Trophy Edit will not work if you have earned Trophies already. GAME Launcher and APP_Home(data) appear if you have installed GTA V Disc (CUSA00411) and OMSK (NPXS29005). PSN User Avatar is only for a PS4 with PSN User.
Note: This Mod is not 100% complete on 4.05 yet, TV & Video icon and Internet-Browser icon is not changed. Thanks to Simple Person and valentinbreiz for dumping app.db (more details HERE and HERE) and testing.

Yesterday I have updated my 1.76 PS4. I have tested the Database and it seem you have to use your own app.db on 4.05, so I decided not to upload version 0.16 yet but you can see my Mod is complete now.

This Tutorial is for PS4 users on 4.05.
If you want a Custom Home Menu it is necessary to edit the own Database, it is just one file you have to edit. The steps in my Tutorial are for the UI Mod alpha 0.16.
Everything you need to make a Custom Home Menu on your 4.05 PS4 is in the PS4 4.05 Modding Pack which you can download above.
PS4 UI Mod 0.16 Alpha Tutorial
1. Start PS4 in Recovery Menu and rebuild Database
2. Open User’s Guide and load the Exploit (redirect via Proxy to https://extreme-modding.de/PS4/4.05/index.html)
3. Send the Payload to activate the Internet Browser (add your IP in activate-web-browser.bat and run it)
4. Open Internet Browser and enable Debug Settings to install OMSK.pkg and restart the PS4
5. Enable FTP Server and copy the app.db (\system_data\priv\mms) to your PC (change your IP in ftp_server.bin)
6. Now you have to edit the Database

7. Copy your modified app.db in the PS4 UI Mod™ 0.16 Alpha folder (\Mod\system_data\priv\mms)
8. Enable FTP Server and copy the content (system_data and user) of the Mod folder to the root of the PS4 HDD

Now you should have a Visual Custom Home Menu on PS4 4.05
